First-Years/Transfers/First-time Athletes Checklist

Email to complete athlete information and medical history will be coming before July 1st.


***All information submitted is completely confidential, and cannot be accessed by anyone other than the Athletic Training Room staff once it is submitted.

___Complete the Insurance Requirement Survey by clicking on the following link
        Verification of Health Insurance
        **You must complete this verification by your deadline for your sport

 **IF YOU DO NOT COMPLETE THE vERIFICATION, THE COLLEGE OF SAINT BENEDICT WILL ENROLL YOU IN THEIR INSURANCE; WHICH COSTS $1,715 FOR A YEAR AS AN ATHLETE.**

___Send the following forms to the address below:

        1. PROOF OF INSURANCE FORM (WITH COPY OF INSURANCE CARD FRONT AND BACK) 
        2. COPY OF HEALTH REPORT FORM, WHICH INCLUDES MEDICAL PHYSICAL (Do not send original).
            ***This is the form that you send to CSB Health Services after you have a physical.  Make a copy of that form.
